Liver Diseases Burden in India

Liver and pancreatic diseases are silent killers, since they frequently don't exhibit symptoms until advanced. Chronic liver disease (CLD) is characterised by a steady decline in the functioning of the liver for longer than six months, resulting in cirrhosis and can cause liver failure, a life-threatening condition.


In India, the burden of liver disease has spiked, with 1 in 5 adults affected by liver diseases such as hepatitis (A, B, and C), cirrhosis, liver cancer, fatty liver, alcoholic liver disease, autoimmune hepatitis, liver abscess, etc. Congenital abnormalities such as biliary atresia affect one in every 10,000 to 15,000 infants annually. In India, more than 2.5 lakh people and globally, 20 lakhs of people die of liver disease due to viral hepatitis (chronic), liver cirrhosis, and hepatocellular carcinoma annually. Liver cancer is currently the 16th most prevalent cause of death worldwide, and cirrhosis is the 11th.


Treatment for these life-threatening diseases mainly involves surgical interventions such as hepatectomy, liver transplant, and medical management. Hepatectomy is a surgical procedure where a part or total amount of the liver (depending on the area infected) is resected or removed. However, many people in India are not in a position to afford the treatment (surgical and medical management), as 52% of the Indian population is under the middle class, and 16.4% is in poverty (2019 stats).

More than 1800 liver transplants are performed annually in India, and more than 50,000 patients are estimated to need liver transplantation each year. In India, more than 96 % of liver disease patients need liver treatment. This implies the unavailability of liver treatment that aids in bringing smiles back to most patients due to financial constraints. 

Pancreatic Diseases Burden in India

The pancreas, which produces the digestive enzyme pancreatic amylase and the hormone insulin, is close to the liver and shares a portion of the common bile duct. A tumour within the pancreas can grow large before it produces any pain or other signs. By the time it shows the symptoms, the tumour has progressed to the liver, lungs, or other areas of the abdomen and cannot be surgically removed. In addition, autoimmune diseases like type 1 diabetes stop producing insulin and might lead to kidney failure, which paves the way for renal and pancreatic transplantation.


In India, the prevalence rate of pancreas disease or pancreatitis is 7.9 cases per one lakh, which includes acute pancreatitis, alcohol-related pancreatitis, diabetes, gallstone pancreatitis, etc. Congenital anomalies include pancreas divisum, contributing 4 to 14% of the incidence rate.


Pancreatic cancer occurs in 0.5-2.4 per one lakh men and 0.2-1.8 per one lakh women. According to a recent study, more than 4.5 lakh deaths globally and more than 12 thousand deaths in India were reported due to pancreatic cancer in 2020. More than 64,000 new pancreatic cancer diagnoses are anticipated worldwide in 2023, along with more than 50,000 fatalities.


The treatment for such fatal ailments includes continuous medical management with insulin, chemotherapy, antibiotics and painkillers. The endoscopic and surgical procedures that include endoscopic retrograde cholangiopancreatography (ERCP), Spyglass lithotripsy, cholecystectomy (removal of gall bladder), pancreatic transplantation, etc. that are highly effective in treating chronic pancreatic conditions in addition to being expensive. These expensive procedures seem to be hard to afford for many people, especially those with low- and middle-income families.

In 2019, as per a study in India, 22 patients underwent pancreatic transplants out of 143 pancreatic disease patients needed a pancreatic transplant. Despite the limited number of pancreatic transplants in India, the waiting time for a transplant is nearly one year.
